Frailty is nearly universal in patients with advanced heart failure receiving LVAD therapy — but it’s not fixed. In an analysis of 2,700+ HeartMate 3 LVAD patients: - 95% were classified as frail at baseline - Frailty was strongly associated with adverse outcomes - Within 3 months, 15% of patients recovered to a non-frail status, which was associated with a significantly lower risk of adverse events. These findings, first seen at ISHLT 2026, suggest frailty in advanced heart failure may reflect reversible physiological vulnerability, not irreversible decline.
