Day 1: Build the value-based care machine 🛠️ Day 2: Make it defensible 🔐 Yesterday’s VBC 301 session with Out-Of-Pocket was all about audit-ready documentation, care gap closure, and moving the work upstream to the encounter. And Roma Patel of Claremedica brought 👏 it 👏 One of the biggest themes of the day: More documentation ≠ better documentation. The goal isn’t a longer note, but rather, a more meaningful one that accurately tells the patient’s story, supports the clinical decisions being made, and can stand on its own under audit. Roma took us from the compliance and coding side all the way down to one patient sitting in front of their PCP showing what happens when fragmented records mean the provider just doesn’t have the full picture. Then Zac Oty picked up that exact patient story and showed us what it looks like when you actually close that loop in the workflow.
