Ad stands with the accusative, not the dative, so it should be Aniam adoratam […] et electam. That should also clear up any ambiguity regarding who is chosen.

The accusative of amans is amantem, not -tum.

Meme is technically correct, but it is an exceedingly rare form. The usual intensifier of me is memet. Whichever you choose, they're both two syllables long.
