Record metrics for incremental tool updates

What changed

Record codex.tools.incremental_updates through session telemetry with an action tag for added, removed, and schema_changed. Definition changes include description edits.

Exclude initial catalogs, unchanged tools, and namespace headers from the counts. Count each removed namespace member even when the removal notice lists only the namespace.

Testing

Extend the namespace transition snapshot test with in-memory metric assertions covering additions, removals, definition changes, and excluded initial or unchanged catalogs.
